You probably already know that using tobacco causes significant health risks: It increases your odds of getting various cancers and coronary diseases, to name just a few. Unfortunately, not everyone is able to kick the habit, even when they know they should. Tooth loss is another issue that can cause trouble for your health, in the form of bone loss, malnutrition, and social or psychological problems. Dental implants are a great way to replace missing teeth — but does smoking complicate the process of getting implants?
The short answer is yes, smoking can make implant placement a bit riskier — but in the big picture, it doesn’t mean you can’t (or shouldn’t) have this procedure done if it’s needed.
Smoking, as you know, has harmful effects in your mouth (even leaving aside the risk of oral cancer). The hot gases can burn the oral cavity and damage salivary glands. Nicotine in smoke reduces blood flow to the soft tissues, which can affect the immune response and slow the processes of healing. At the same time, smoking promotes the growth of disease-causing oral bacteria.
How does this affect dental implants? Essentially, smoking creates a higher risk that implants may not heal properly after they are placed, and makes them more likely to fail over time. Studies have shown that smokers have an implant failure rate that’s twice as great as non-smokers. Does this mean that if you smoke, you shouldn't consider implants to replace missing or failing teeth?
Not necessarily. On the whole, implants are the most successful method of replacing missing teeth. In fact, the overall long-term survival rate of implants for both smokers and non-smokers is well over 90 percent — meaning that only a small percentage don’t work as they should. This is where it’s important to get the expert opinion of an implant specialist, who can help you decide whether implants are right for your particular situation.
If you do smoke, is there anything you can do to better your odds for having a successful dental implant? Yes: quit now! (Implants are a good excuse to start a smoking-cessation program.) But if you can’t, at least stop smoking for one week before and two weeks after implant placement. And if that is not possible, at least go on a smoking diet: restrict the number of cigarettes you smoke by 50% (we know you can at least do that!) Try to follow good oral hygiene practices at all times, and see your dentist regularly for checkups and cleanings.
Modern restorations for severely damaged or missing teeth are truly remarkable. Although the type of restoration may differ — dental implant, bridge, or veneer — the end result is a life-like facsimile that matches the shape and color of your natural teeth.
To achieve this result, though, the new crown or veneer requires fabrication in a dental laboratory, a meticulous process that may take a few weeks. In the interim, we often install temporary crowns. These help in a number of ways: because we’ve prepared the teeth for final restoration by removing some of the tooth structure, the temporary crown protects and stabilizes the teeth, reduces sensitivity and helps maintain gum health. Temporary crowns also enable the patient to present a more natural smile while waiting for the permanent restoration.
Temporary crowns are typically manufactured to fit a wide range of patients, similar to an “off-the-rack” suit from a clothing store. In recent years, though, customized “tailored” temporary crowns designed specifically for an individual patient have grown in popularity among dental professionals as well as patients.
In creating a customized temporary crown, we first perform a smile analysis similar to one used for a permanent restoration. After a careful assessment of your mouth, we would then make recommendations about the elements to include in the temporary crown, including shape and color. We would also factor in your desires and concerns into the final design. Working together with the dental laboratory, we would then have the temporary crowns produced and ready to apply as soon as we complete the preparatory work.
Customized temporary crowns also serve another important purpose as a kind of “dress rehearsal” for the permanent restoration. This gives you an opportunity to “try out” the smile you’ll have with the permanent restoration in your daily life. We can then use your experience to make adjustments to the permanent crowns before final production.
While customized temporary crowns involve more effort and expense than the traditional, the benefits are worth the added cost, especially if you’re involved with an extensive “smile makeover” procedure. Not only will you look better while your permanent restoration is under construction, your temporary look will give you added confidence that your future smile is right for you.

Tooth decay is a primary cause of tooth damage and loss, with annual treatment costs in the billions of dollars. It arises mainly from oral bacteria, which proliferates in the absence of effective oral hygiene. There are, however, other risk factors besides poor hygiene that could make you more susceptible to this disease.
Many people, for example, have genetically inherited deeper grooves (fissures) and depressions (pits) than the average tooth anatomy. These may be harder to reach with a toothbrush and can become havens for bacterial plaque. Others may have health conditions that indirectly affect the mouth: bulimia or anorexia, psychological conditions that involve self-induced vomiting, or GERD, gastro-esophageal reflux disease, in which stomach acid could regurgitate into the mouth. These conditions could result in a highly acidic mouth environment.
Some medical and — ironically — dental treatments could also increase your tooth decay risk. Some medications can reduce saliva flow, which inhibits acid neutralization and re-mineralization of enamel. Retainers, braces, bite guards or other dental appliances may also reduce the saliva wash over teeth, and can make brushing and flossing more difficult.
There are also risk factors that result from our lifestyle choices. Eating a lot of foods rich in sugars and other carbohydrates, for example, or acidic beverages like soda, energy or sports drinks contributes to the rise of bacteria in our mouths.
There are ways to reduce the effects of these risk factors. In addition to a daily habit of effective brushing and flossing, you should also include semi-annual cleanings and checkups at our office a part of your routine. If you have genetic, medical or dental issues that are out of your control, we can discuss solutions, such as alternatives to medications or different techniques for cleaning around dental appliances. For lifestyle-related factors, you should consider removing the habit or modifying it: for example, snacking at specific times or drinking acidic beverages only at mealtime.
While tooth decay is a serious, destructive disease, it is highly preventable. Addressing all your risk factors, not just hygiene, will reduce your chances of having it.
